Subject: Keystone Civic Association Wate rqy

Dear Ms. Adams:

As you were made aware at the February 25, 1993, Keystone
Civic Association meeting, the Keystone Civic Association is
conducting an on-going water well survey in the area of
northwest Hillsborough County. On March 4, 1993, members of
the Keystone Civic Association provided the District with the
initial results of the water well survey. As explained by the
Association members, the water well survey is being conducted
through mailed questionnaires and all of the responses to the
survey questions are being entered onto a spreadsheet
database. In addition, the Association has also catalogued
comments which are submitted with the responses to the survey.
The survey addresses primarily water quality issues but does
include a question regarding dry wells. None of the survey
respondents names have been provided with the information,
however, their addresses are provided in the spreadsheet
database.

Enclosed for the Authority's information and disposition is a
copy of the entire packet submitted by the Keystone Civic
Association members. Included please find a blank copy of the
actual survey form being used, a copy of the survey form with
the initial results to questions answered by 270 respondents,
a computer disc containing the spreadsheet database file in
Lotus format (SWFWM.WK1) and the catalogued comments in ASCII
format (SWFWM.TXT), and a copy of the code table for the
spreadsheet. Any questions the Authority may have regarding
the survey should be addressed to Mr. Warren B. Nelms,
Keystone Civic Association, at his home telephone number of
(813) 920-6858.

Explaination of Field Name & Instructions

Number of survey
Leave blank

Any Logical field enter Y for Yes or N for No
if the survey says Y or check mark. If the
survey is blank, leave that field blank.
Not on survey: if there are no changes enter "Y"
EXCEPTION: If they have entered comments
in any of these character fields, they should have
put a check in the Logical field. Therefore, enter
a Y for the Logic.

Example: The survey says "smell started 1989" here
so there should be a "Y" in ROTEGG_YN"

There are many times comments here and in other
places. Save them for the memo in the MISCELLAN
(last) field.

When costs are given as $xx /month, multiply the
xx times 12 (months) times 10 (years life) for the
best guess at equating to the other surveys. Combine
IRON_COST with CONDCOST before the multiply
and enter the single number in field 31.

If there is a number in Field 40, put a "Y" here. Better.
if you can tell it's a lake, put a "L", a pond. "P", etc.

2, 3, 4, whatever
Enter "J", "S", or "C" for type of pump
Enter "B" for Bored (drilled) or a "D" for Driven

If they entered in feet, divide by 5280 and enter as
nearest one decimal place.

When cursor is in this Memo field, enter Ctrl Home
to start writing memo. Type in all comments from
survey with enough info to show where the comments
are coming from. Enter your comments within brackets
Example: [NAME NOTE: This survey needs special
attention.] When through with memo, Ctrl W gets
you back to the MISCELLAN field. Enter or down
arrow saves this record and starts you into next record.
