Abstract:
Background: Over 30 years ago, Mishkin and Ungerleider proposed two visual processing streams underlying spatial (occipitoparietal) and object-based perception (occipitotemporal). In this study, this well-known dissociation was applied to individuals with Parkinson's disease (PD), a dopaminergic depletion disorder affecting frontal-subcortical systems. We hypothesized that PD subtypes could be distinguished based on performance on spatial and object-based cognitive tasks. Further, we hypothesized that performance on these tasks would be linked with other cognitive functions activating neuroanatomically similar regions, such that spatial task performance (fronto-parietal) would be linked with executive function and object-based task performance (temporal lobe) would be linked with episodic memory function. The identification of cognitive subtypes may have implications for improved understanding of the nature, trajectory, and mechanisms underlying cognitive decline in PD. Methods: A cross sectional design was used, including neuropsychological data on 336 PD patients obtained from the University of Florida Center for Movement Disorders and Neurorestoration INFORM database. To examine whether PD patients could be classified into visuocognitive subgroups, two analyses were used: hierarchical cluster analysis and psychometric cutoff subtyping. Performance on executive and memory tasks across subgroups was examined via a series of one-way ANOVAs. Results: Four different subgroups arose from the cluster analysis and psychometric cutoff classification approaches: 1) a subgroup with spatial impairments, 2) a subgroup with object-based impairments, 3) a subgroup with unimpaired visuocognitive performance, 4) a subgroup with impaired spatial and object-based performance. The spatial-impaired group performed significantly worse than the object-impaired group on executive measures. In contrast to expectation, the object-impaired and spatial-impaired groups did not differ on a temporal lobe memory task. Conclusions: Hypotheses were partially supported. Distinct visuocognitive profiles were identified in a PD sample, mirroring the theoretical dissociation of visual processing. Findings that spatial abilities were linked to frontal-executive function suggest that spatial tasks have high executive load. Contrary to research suggesting that object-based abilities are dependent on temporal lobe activation, object-based impairments were not linked to reduced episodic memory. Findings are discussed in terms of theoretical views on mechanisms underlying visuocognitive impairment and trajectory of mild cognitive impairment (MCI) in PD.
