Dear Daryl Ellison,

The project under my force account direction is complete. I have enclosed
the Certificate of Substantial Completion, the Completion Contract with the
State of Florida release of lien, the general contractors release of lien, and
other supporting documentation.

As you know the project was a combination of fixed bid and cost plus. The
fixed bids were for the electrical ($10,860) and mechanical ($10,535), see
enclosed memos dated October 20, 1988.

The cost plus not to exceed contract was with Len Weeks Construction,
Design, Development for a total of $98,000. There were other miscellaneous
expenses charged against the project that did not exceed $3,000.

The Historic St. Augustine Preservation Board has received and filed all
necessary warranties, guarantees, and operating instructions for the equipment
installed under this contract.

If there is any other documentation your office requires, please contact
the Historic St. Augustine Preservation Board.

Government House, St. Augustine, Florida

Dear David:

Substantial completion inspection was conducted on May 18,
1989, at the referenced site. You, Mr. Len Weeks, General
Contractor, and I were present. Following the inspection,
AIA Substantial Completion forms were completed by the
Owner, Contractor, and Architect. It was agreed that the
General Contractor will provide a general Release of Liens
at final completion for work under his contract; the Owner
will obtain releases from subcontractors under separate
contracts.

Before conducting the inspection we reviewed the general
status of the project and determined the following:

1. At substantial completion $4,989.69 remains in the
construction budget. Cost projections indicate this money
will be expended by final completion.

2. Work originally included in the contract documents
but not completed is as follows: utilization of the allow-
ance for carpet, and completion of the countertop and finish
plumbing in the Staff Room (plumbing roughing-in was com-
pleted). Work beyond the original scope of work, listed
below, was substituted for these items.

3. Work completed that is beyond the scope of the
original contract documents includes the following:

a. In the auditorium, removal of the stage plat-
form, all seating, seating risers, interior partitions, and
auditorium ceiling. None of these items were in compliance
with current codes. In addition, fire-rated wallboard on a
metal suspension system was installed at exposed areas of
the ceiling, and new utility lighting was provided for
interim use.

b. The security system was extended to serve the
restored East Wing.

c. Original door floor closer at the south doors
of the East Wing were rebuilt, and additional hardware was
provided in certain areas where existing hardware was not
reusable. Provisions were made for future installation of
magnetic hold-open devices for new doors from the lobby into
the East Wing. These items were beyond the scope of the
original allowance.

d. Marble veneer was replaced on lobby walls, and
fire doors in lobby were trimmed to appear compatible with
original doors.

e. The discovery of historic footings below the
lobby stair at the west end of the East Wing resulted in the
relocation -of the air-handling unit from a proposed new
tunnel to the existing tunnel.

The substantial completion punch list follows. The correc-
tions noted in Item 1 are not the responsibility of the
General Contractor because electrical and mechanical work is
contracted separately by the Owner. The Owner shall ensure
that the corrections required for Item 1 are made by the
responsible subcontractors. All other items are the respon-
sibility of the General Contractor.

1. Mechanical and electrical items:

a. Correct items noted on final electrical and me-
chanical inspections by DGS consultants.

b. After the system runs on the cooling cycle,
water appears on the trench floor. Either the coils are
freezing due to restricted air flow, the sump pump is inop-
erable, or the condensate pan leaks. A representative from
the mechanical contractor was at the site investigating this
problem when the inspection was completed.




















2. Complete the installation of the display cases and
install the wood stringer at the steps down into the East
Wing.

3. Complete installation and repair of exterior door
closer, historic hardware, weatherstripping, and push
plates.

4. Patch stucco in exterior wall around electrical
conduit that served relocated condensing unit previously
located on the balcony.

5. Provide a final copy of door hardware schedule to
Owner.

6. Provide Owner with a list of paints and stains
utilized on the project, including manufacturers' product
names.

7. Provide general cleanup.

8. Provide a positive means of attaching flexible ducts
to the tops of display cases.

9. At west end of new corridor behind display cases,
replace existing cylinder lock operated by key from lobby
side with cylinder lock operated by key from lobby side and
thumb latch from corridor side.

10. Install hooks and eyes on display case doors.

11. If additional funding becomes available, astragals
and closing coordinators (or removable mulls) should be
provided at existing pairs of rated doors opening into the
lobby. An astragal (or removable mull) will be provided at
the new pair of rated doors from East Wing into lobby when
funding becomes available for magnetic hold-open devices.
However, it is preferable to delay this work until a com-
plete study of egress requirements for the entire building
in completed in the future. Neither the study of overall
egress requirements nor the complete upgrade of lobby egress
hardware was part of the scope of work of this project.
