You are hereby notified by the Director of the Division of Hotels and Restaurants. Bureau of Elevator
Inspection that he has been presented certain evidence which, if true, is good and sufficient cause for
him, pursuant to Chapter 399, Florida Statutes, to assess an administrative fine against you.

As the owner or lessee of the elevator, it is your responsibility to ensure the safe operation and proper
maintenance of the elevator after it has been inspected by a State Elevator Inspector.

Please be advised that you have 30 days from the date that the inspection report was issued (indicated
above) to notify the Division of compliance. Failure to comply within the 30 day period will result in
the issuance of a Notice to Show Cause why the Division should not take further administrative action.

Under Chapter 399.105(4), F.S.. any elevator owner who fails to comply with an order (inspection report)
within 30 days after its issuance is subject to an administrative fine set by the Division in an amount
not greater than $500.

If this elevator is maintained under a service maintenance contract according to the Bureau's records, it
is suggested that you immediately notify the maintenance company of the violations which appear on
the inspection report.

You may avoid further administrative action by providing the Bureau with the following information:

1. Owner's copy of the referenced inspection report, properly signed, certifying the violations have
been corrected or by signing in the compliance space provided below,
OR
2. Provide proof of compliance, such as, a letter certifying the violations have been corrected from
the elevator maintenance company, general contractor, firm or person who completed repairs.

IMPORTANT: PLEASE NOTE THAT PROVISION NO. 1 OR NO. 2 MUST BE PROVIDED TO THE DIVISION.

I hereby certify that the violations listed on the Elevator Inspection Report referenced in this order have
