Stephen Benninger Owner of Heritage Mall on St. George Street Notified Our Dept. On 08/21/97 That Orkin Pest Control would be tenting his mall on 09/21/97 through 09/26/97
and he was told to notify all surrounding neighboring buildings of such. Mr. Ben Smith's property adjoins the mall and therefore must vacate the premises during the tenting
for health and safety reasons. The fumigation will be done by a subcontractor, Pioneer Pest Control from Deland, FL. On 08/29/97 David Brandt spoke with Ben Smith to find
out if Mr. Benninger had been in contact with him regarding his need to vacate his home. Mr. Smith said that no one had contacted him. Mr. Brandt brought
him up-to-date and told him that he would have Mr. Benninger give him a call. A letter was mailed to Mr. Smith from David Brandt on 09/02/97 via certified mail requesting that he sign a form
called, Customer's Duties In Preparing For Tent Fumigation. Mr. Smith was to read, sign and retum to us this form. Mr. Ben Smith called for David Brandt on Tuesday, September 16th
to ask a question having to do with his duties as a tenant to cut tree limbs, fix fences, etc. He was told that Mr. Brandt was out of town until the 19th but it was suggested that he
call Mr. Binninger to ask him any questions he might have. Mr. Smith stated that he would send the form back in once his questions were answered. An Agreement form was written
by the City Attorney and Mr. Smith was sent an original of this Agreement for signing and returning. This was delivered by Ray Plate on Wednesday, September 17th. A letter was
attached requesting that he return to our department ASAP.

Ms. Clough's Hot Water Heater Was Replaced On 02/13/98. Week of 02/16/98 Mike Griffin, Fire Chief Jay Kennedy & David Brandt is trying to get special permit approved
to allow Ms. Clough to utilize the upstairs portion of building as her working studio. The winding staircase is a safety issue and until the City can approve the use of the
staircase by Ms. Clough alone, she cannot get her occupational license to open her shop for business.

Ms. Clough would like to plant some plants on the property at her expense (Mike Griffin's department does not have to approve). She would like to paint the interior of the building (Mike Griffin's department will have to pre-approve).
She would also like to add a sign at the road advertising her shop (Mike Griffin's department will have to approve lettering).

The State Fire Marshall Sent A Letter To David Brandt's Attention On Friday, 02/27/98 Notifying Him That 57 Treasury Has Been Approved As Is -
For Manila To Open Her Place of Business But Only If She Is The Only One Utilizing the Upstairs. A Copy of This Letter Was Mailed To Manila On
Monday, 03/02/98.

Ms. Clough Had A/C System Checked By Powell A/C On 04/15/98 794-2665.
