Florida will receive a lump sum of the undesignated dollars awarded to the United Way Agencies participating in

will receive their designated and percent of undesignated dollars directly.

Step I Select payment method
Indicate your method of payment by selecting payroll deduction, check/cash, or direct bill. If you select payroll deduction, please sign your pledge card.
Step 2 Indicate Recognition Method
If a leadership giver ($500.00 or more), indicate how you would like to be recognized. If not, proceed to Step 3.
Step 3 Agency Designation (see 2008 UFCC brochure for agency information).
If designating to a specific agency, select your agency's code below. Enter the amount you would like to designate.
