The University of Florida Board of Trustees has established the following Ethics
Policy for the University of Florida:

All trustees, officers, and employees of the University of Florida will adhere to the
highest ethical and business practices. All actions of the trustees, officers, and
employees of the University shall be in compliance with the law, including the
Code of Ethics for Public Officers and Employees, Chapter 112, Florida Statutes,
and the University of Florida Guidelines, Policies, and Procedures on Outside

The trustees and the senior officers of the University will affirm in writing on an
annual basis their commitment to this Ethics Policy.

The Ethics Advisor for the University of Florida is the Vice President and General
Counsel, 123 Tigert Hall, Post Office Box 113125, Gainesville, Florida 32611,
352-392-1358 office or 352-392-4387 facsimile.

July 1, 20 through June 30, 20

Are there any entities of which you or a relative* are an employee, officer, trustee, or
member of the board of directors, or have ownership interest of 5% or more, that have
entered into business transactions with the University of Florida or with a
University of Florida direct support organization, health services support organization,
faculty practice corporation or affiliated corporation**?


If yes, please describe the nature of your interest in the entity (or entities) and the
business transaction (or transactions).

Annual Ethics Affirmation

To the best of my knowledge, the foregoing is true and correct and all the
transactions described were allowable and disclosed. In addition, I affirm my
obligations under the University of Florida Ethics Policy.





* "Relative" includes the following: father, mother, son, daughter, brother, sister, uncle, aunt, first cousin, nephew,
niece, husband, wife, father-in-law, mother-in-law, son-in-law, daughter-in-law, brother-in-law, sister-in-law,
stepfather, stepmother, stepson, stepdaughter, stepbrother, stepsister, half brother, half sister, grandparent, great
grandparent, step grandchild, step great grandchild, person who is engaged to be married to the Trustee or with
whom the Trustee intends to form a household or any other person having the same legal residence as the Trustee.

** "Affiliated corporations" are: UF Jacksonville Healthcare, Inc., Faculty Clinic, Inc. d/b/a UF Faculty
Clinic/Jacksonville, Shands Teaching Hospital and Clinics, Inc. and Subsidiaries, University of Florida Healthcare
Education Insurance Co., Baby Gator Child Care Center, Inc., and University Village Apartments, Inc.
