In consideration of sums received and to be received for providing treatment for the above-stated wood
destroying insect at the premises specified above, ORKIN guarantees, subject to the Terms and Con-
ditions appearing on the reverse side of this Guarantee and on the Service Order bearing the above-
stated number:

To inspect the premises treated, as described in said Service Order.


To apply any necessary additional treatment to such premises, AT NO EXTRA COST, if infesta-
tion of the above wood destroying insect is found in the premises during the period that this
Guaranty remains in force. This Guarantee is limited to retreatment only and in no way, implied
or otherwise, covers damages and repairs to the structure or contents.



RENEWAL AND GUARANTEE PERIOD

SThe initial period of this Guaranty shall commence on the date of initial treatment shown above. In addition
to this initial period you may thereafter, at your sole option, renew this Guaranty annually for the number of
years indicated above. The Guaranty will be renewed annually upon receipt by the Company of the above-
stated annual renewal premium on or before the renewal date of each subsequent year,
Upon receipt of the annual payment each year, the Company will inspect the premises. If the annual renewal
payment is NOT made on or before the renewal date of any subsequent year, this Guaranty shall terminate as
of the renewal date on which said payment is due. This Guaranty covers the premises as of the date of actual
treatment and in the event the premises are structurally modified, altered, or otherwise changed after the
date of the initial treatment, this Guaranty shall terminate, unless a prior written agreement shall have been
entered into between the Owner and the Company to reinspect the premises, provide additional treatment if
necessary and/or adjust the annual renewal premium.

TO THE UNDERSTRUCTURE OF THE PREMISES AND THE FIRST TOP-FLOORING BUT NOT INCLUDING AREAS ABOVE
THE FIRST FLOOR LEVEL) FOR POWDER POST BEETLE CONTROL AND WILL ISSUE A CONTROL GUARANTY.

SHOULD REINFESTATION BE FOUND IN THE TREATED AREAS, ORKIN WILL, AT NO EXTRA COST, PROVIDE ADDI-

(FUMIGATION TREATMENT ONLY)
SUBJECT TO THE GENERAL TERMS AND CONDITIONS, ORKIN WILL FUMIGATE THE STRUCTURE FOR THE CONTROL
OF DRYWOOD TERMITES, POWDER POST BEETLES AND WOOD BORERS, AND WILL ISSUE A CONTROL GUARANTY
WHICH WILL PROVIDE FOR RETREATMENT AT NO EXTRA COST, SHOULD A REINFESTATION BE DISCOVERED
DURING THE PERIOD THE GUARANTY IS IN FORCE.

THE PURCHASER FURTHER UNDERSTANDS THAT THIS PROTECTION MAY BE RENEWED ANNUALLY BY WRITTEN
MUTUAL AGREEMENT, FOR A MAXIMUM GUARANTY PERIOD OF FIVE (5) YEARS. THE PURCHASER FURTHER UN-
DERSTANDS THAT ORKIN'S LIABILITY UNDER THIS AGREEMENT IS LIMITED TO RETREATMENT ONLY AND IN NO
WAY, IMPLIED OR OTHERWISE, IS RESPONSIBLE FOR DAMAGES OR REPAIRS TO THE STRUCTURE OR CONTENTS.

GENERAL TERMS AND CONDITIONS

By specific reference hereto, the Purchaser understands that the service Order is hereby made a part of the guarantee and
applies without exception.
1. It is agreed that under this contract, ORKIN is not responsible for the repair of visible damage existing as of the date of
this contract except as such damage is described on the Graph and Specification Sheet and for which a specific charge
for the repair of same is being made as indicated on the face of the Service Order.

2. It is possible that damage may, as of the date of this contract, exist in unexposed areas of the structure or in areas
which are inaccessible to visual inspection. For this reason, ORKIN cannot guarantee that the damage disclosed by
visual inspection of the premises (and which is indicated on the Graph and Specification Sheet) represents the entirety
of the damage which may exist as of the date of initial treatment. It is specifically understood, therefore, that ORKIN
shall not be responsible for the repair of any damage which existed in areas or in structural members which were not
accessible for visual inspection as of the date of this contract.
3. If moisture and/or structural conditions which are conducive to Subterranean Termites are subsequently found to exist,
then Orkin shall be relieved of any and all liability for damage repairs.
4. Structural or mechanical defects which result in water leakage in interior areas or through the roof or exterior walls of
the premises may destroy the effectiveness of ORKIN'S treatment, thereby permitting infestation to continue after the
date of initial treatment. If such a condition is discovered, it is agreed that the customer will be responsible for making
such repairs as are necessary to correct the structural or mechanical defect and ORKIN will, upon completion of said
repairs, provide additional treatment deemed necessary by the Company to control the infestation in the area.
5. Any claim for breach of any Guaranty shall be made forthwith in writing to said ORKIN EXTERMINATING COMPANY, INC.,
2170 Piedmont Road, Atlanta, Georgia, 30324.

6. No suit shall lie hereunder unless the provisions of Paragraph 5 have been complied with and unless brought within one
(1) year after the making of said written demand.

7. ORKIN's liability shall be terminated should ORKIN be prevented from fulfilling its responsibilities under the terms of
the Service Order or the Guaranty by reason of Acts of War, whether declared or undeclared, Acts of any duly constituted
Government Authority, strikes or Acts of God.
8. This agreement covers the premises diagramed on the Graph and Specification Sheet bearing the Service Order Number as
of the date of actual treatment, and in the event the premises are structurally modified, altered or otherwise changed
after date of initial treatment, this agreement shall terminate unless a prior written agreement shall have been entered into
between the Owner and the Company to reinspect the premises, provide additional treatment and/or adjust the Annual
Renewal Fee.
9. It is agreed and understood that installment payments due hereunder shall be made payable to ORKIN EXTERMINATING
COMPANY, INC., its successors or assigns. In the event of default of any installment, ORKIN or its Assignee may
declare the entire balance of this agreement immediately due and payable. Purchaser agrees to pay a "late charge" of
5% of the amount of the installment in default (if permitted by law) and if the services of an attorney be employed for
the enforcement of any of the obligation of the Purchaser, or rights of the Seller either by suit or otherwise. Purchaser
agrees to pay attorney's fees of 15. of the unpaid balance of the contract.

10. It is understood and agreed between the parties that this instrument, the Service Order and Graph constitutes the com-
plete agreement between the parties and that said agreement may not be changed or altered in any manner, oral or other-
wise, by any representative of ORKIN unless alteration or change be in writing and executed by a corporate officer of
ORKIN EXTERMINATING COMPANY, INC. under the corporate seal.

PREPARATION

P/ Pilot lights on all equipment will be turned off.
Open all furniture drawers, cabinets, attic vents, etc. to speed
/up gas penetration and ventilation.

SEALING

_ Completely enclose building in Orkin's special vinyl coated nylon
tent.
Paper seal with masking tape and specially treated paper.
Protect roots of shrubbery as well as possible by sealing base of
tent with ^ lawn sand fill dirt sand snakes.

Place warning signs about perimeter of tent.
+ Post an alert guard on the premises throughout fumigation operation.
Registered fumigator will release fumigant.
SNotify County Board of Health, Fire and Police Departments having

I Ventilate building thoroughly and test with appropriate detector, to see
that all harmful gas concentrations have dissipated.
Sweep walks and stoops and scatter sand (lawn sand) away from base of
building. Extra sand or dirt will be left for customer's use. Post written
notice on front door indicating date and time building will be safe for
re-occupancy.

Upon completion of the work, and payment for our services in the amount

Insect Damage Guarantee. This Guarantee is backed by our corporate assets
of more than $50, 000, 000. 00.

This Guarantee provides for an annual reinspection to assure that these insects
do not return. As the owner of an Orkin-treated building, you may continue
protection from year to year for five years, assuring you freedom from damage
over this period of time.

At the end of each five year period, the property will be reinspected again, and
the Guarantee may be extended for another five years, by mutual agreement.

The cost of Orkin's Guarantee is modest a nominal annual inspection renewal

TERMITE CONTROL
DETAILED SPECIFICATIONS



1. Remove all debris containing cellulose and haul from premiss.

2. Trench or rod the soil adjacent to the inside foundation wallJ approx-
imately 6 inches deep, treat trenches with an application of patented
liquid ORKIL.

3. Trench or rod around piers approximately 6 inches deep and treat with
patented liquid ORKIL, backfill.

4. Trench around chimney base approximately 6 inches deep and saturate
trench with patented liquid ORKIL.

5. Drill the piers at proper intervals and flood void area with patented
liquid ORKIL.

6. /Drill chimney base at intervals and flood void area with patented
liquid ORKIL.

7. _Drill cement block foundation wall at proper intervals and flood voids
with patented liquid ORKIL.

8. Drill completely through foundation walls at approximately 16 inch
intervals and pressure treat soil under perimeter of slab with special
hollow treating rods. Refill holes with cement.

9. Drill brick veneer at approximately 8 inch intervals, completely

OF DRYWOOD TERMITES, POWDER POST BEETLES AND WOOD BORERS, AND WILL ISSUE A CONTROL GUARANTY
WHICH WILL PROVIDE FOR RETREATMENT AT NO EXTRA COST, SHOULD A REINFESTATION BE DISCOVERED

GENERAL TERMS AND CONDITIONS

By specific reference hereto, the Purchaser understands that the service Order is hereby made a part of the guarantee and
applies without exception.
1. It is agreed that under this contract, ORKIN is not responsible for the repair of visible damage existing as of the date of
this contract except as such damage is described on the Graph and Specification Sheet and for which a specific charge
for the repair of same is being made as indicated on the face of the Service Order.

2. It is possible that damage may, as of the date of this contract, exist in unexposed areas of the structure or in areas
which are inaccessible to visual inspection. For this reason, ORKIN cannot guarantee that the damage disclosed by
visual inspection of the premises (and which is indicated on the Graph and Specification Sheet) represents the entirety
of the damage which may exist as of the date of initial treatment. It is specifically understood, therefore, that ORKIN
shall not be responsible for the repair of any damage which existed in areas or in structural members which were not
accessible for visual inspection as of the date of this contract.
3. If moisture and/or structural conditions which are conducive to Subterranean Termites are subsequently found to exist,
then Orkin shall be relieved of any and all liability for damage repairs.
4. Structural or mechanical defects which result in water leakage in interior areas or through the roof or exterior walls of
the premises may destroy the effectiveness of ORKIN'S treatment, thereby permitting infestation to continue after the
date of initial treatment. If such a condition is discovered, it is agreed that the customer will be responsible for making
such repairs as are necessary to correct the structural or mechanical defect and ORKIN will, upon completion of said
repairs, provide additional treatment deemed necessary by the Company to control the infestation in the area.
5. Any claim for breach of any Guaranty shall be made forthwith in writing to said ORKIN EXTERMINATING COMPANY, INC.,
2170 Piedmont Road, Atlanta, Georgia, 30324.
6. No suit shall lie hereunder unless the provisions of Paragraph 5 have been complied with and unless brought within one
(1) year after the making of said written demand.
7. ORKIN's liability shall be terminated should ORKIN be prevented from fulfilling its responsibilities under the terms of
the Service Order or the Guaranty by reason of Acts of War, whether declared or undeclared, Acts of any duly constituted
Government Authority, strikes or Acts of God.
8. This agreement covers the premises diagramed on the Graph and Specification Sheet bearing the Service Order Number as
of the date of actual treatment, and in the event the premises are structurally modified, altered or otherwise changed
after date of initial treatment, this agreement shall terminate unless a prior written agreement shall have been entered into
between the Owner and the Company to reinspect the premises, provide additional treatment and/or adjust the Annual
Renewal Fee.
9. It is agreed and understood that installment payments due hereunder shall be made payable to ORKIN EXTERMINATING
COMPANY, INC., its successors or assigns. In the event of default of any installment, ORKIN or its Assignee may
declare the entire balance of this agreement immediately due and payable. Purchaser agrees to pay a "late charge" of
5% of the amount of the installment in default (if permitted by law) and if the services of an attorney be employed for
the enforcement of any of the obligation of the Purchaser, or rights of the Seller either by suit or otherwise. Purchaser
agrees to pay attorney's fees of 15% of the unpaid balance of the contract.

10. It is understood and agreed between the parties that this instrument, the Service Order and Graph constitutes the com-
plete agreement between the parties and that said agreement may not be changed or altered in any manner, oral or other-
wise, by any representative of ORKIN unless alteration or change be in writing and executed by a corporate officer of
ORKIN EXTERMINATING COMPANY, INC. under the corporate seal.

The termite is the most destructive of all wood eating insects.
They are tiny pests that have been on earth for more than 55
million years.
There are many species of termites, but the type to be con-
cerned about in this area is the subterranean termite which
has its headquarters underground and causes 9500 of all ter-

THE TERMITE FAMILY


Termites are "social" insects and they are highly organized ... each performing specific
duties.
The termite colony is the nest or termitary. Here live the King and Queen, nymphs,
workers, soldiers and immature reproductive.
The King and Queen are the active reproductive, and perform no other functions.
The Queen produces enormous numbers of eggs... sometimes thousands a day. The
Worker termite is responsible for all the damage to your property. Its function is to pro-
vide food for the entire colony. The Soldier termite defends the colony from its enemies
- chiefly ants. The Winged Reproductives swarm in great number in the spring and fall
to start new colonies. They are often mistaken for flying ants.
A single termite colony can contain as many as 250,000 termites and have been known
to live for more than 25 years!
