TERMS AND CONDITIONS OF THIS PERMIT AIE AS FOLLOWS:

1. That a statement in the iMplication and in supporting dau arn nv e and accurate and
based upon the best information available, and that ail conditions set forth hermin wll
be compiled with. If any of the statements in the application and in the suppodn data
are found to be untrue and inaccurate, or if applcant fa.ls to comply with alW of the
conditions st forth herein, then this Permit shad become nul and void.
This Permnnit is redicated upon the assertion by applicant that the use of water applied
for and ar e: is and coninuss to be a reasonable beneficial us u as defined in Section
373.0195). Florida Statutes, is and continues to be consistent with the public interest,
and wil not interfere with any lea usof water existing on the date this Permit is ranted.
3. In wanting this Pwrmit, SWFWMD has= by regulation, reserved from use by applicant,
water in Such locations and quantities for such seasons of the year, as it determines may
be required for the protection o rfish and wildlife and the public health and saery.
Such reservation are subject to periodic review and revision in light of changed conditions.
4. Based upon the application ad suppotnin documents, SWFWMD fnds that the
pplicant's use of water was in existing before Janury L, 1975 at the rate of

Nothing in this permit should be construed to limit the authority of Southwes Florida
Water Management District to declare water shortages and issue orders puCuant to
Section 373.175, Florida Statutes, or to formulate a olan for implementaion dunni
periods of water shortage pursuit to Se:aion 373.246, Florida Statutes.
6. This Permit authorizes the ap ttftnaamed above to make a combined

8. The use of said water ts nstrfct d to the use classification set
forth above. Any change In the usa of said water will reuire a
modification of this Pewrft.

9. In the event an emergency water shortage should be declared, the
district may alter, modify or declare to be inactive, all or parts
of this Permit. An authorized Oistrict Representative may, at any
reasonable time, enter the property to inspect the facilities s and
may require that this Permit be shown.
10. There shall be no runoff from the property as a result of the.
wi thdrawal permitted herewith.

S11. The Oistrict may, at a future date, establish a minimum water level
in the aquifer or aquifer hydrologically associated with these
withdrawals, which may require the permitted to cease withdrawal from
these groundwater sources at times when water levels fal l below these
minimums.

12. Applicant will notify the district immediately upon permanent termi-
nation of withdrawals as permitted herein.
