) U.S. DEPART~EJI' OF HEALTH, EDUCATION, AND WELFARE Social and Rehabilitation Service March 1968 I.'-'DICAL A'.-ISTATICiH FII"11,'7 I'7RD UTIDFJR THE PUBLIC ASSI!ETArCE TITLES OF THE SOCIAL SECURITY ACT 1969 AUGUST 1967 Introduction For the middle month of each calendar quarter State agencies receiving grants-in-aid for public assistance report the number of people who received medical assistance and the amount of payments to medical vendors or to health-insuring agencies. I/ This release presents sum- maries of the data reported for August 1967. The tables cover medical vendor payments under all the federally aided public assistance programs under the Social Security Act. Separate data are shown, nationally and by State, for recipients and payments under medical assistance programs under title XIX of the Act and for recipients and payments under pro- visions for medical vendor payments under titles I, IV, X, XIV, and ) XVI. Statistical Summary About 4.2 million people had payments for medical care made on their behalf in August 1967. For 3.1 million the State agency or its fiscal agent made the payments to medical vendors and for 1.6 million the payments took the form of premium or per capital amounts paid into various health-insuring systems, principally the Social Security Administration system. For some people--about 430,000--both kinds of payments were made. Total expenditures for bills and premiums paid during August came to $256 million, of which only $10.3 million--or 4 percent--went for premiums to insurance systems. Title XIX.--Nine-tenths (90%) of the August expenditures were made by States operating medical assistance programs under title XIX. These States also accounted for nine-tenths (90%) of the people for whom 1/ Limitations of monthly data and a description of the data reported are provided following the statistical summary for the report month. payments were made to medical vendors and for about two-fifths (37%) of those on whose behalf monthly premium payments were made to various types of insurance funds. Note: Because .premium payments comprise so small a proportion of total expenditures, yet -are'made every month for a sizeable group, including many for whom no medical service actually is provided, an analysis which excludes these payments and the people on whose behalf they are made, is more meaningful thn dhie in which they are included. These data are therefore excluded fr4 the summary which follows; they are, however, show in the accom i ing tables. Distribution of reCipients and payments by basic eligibility factor.--As indicated above, recipients of medical assistance are classified in five groups on the basis of the eligibility factor that enables them to qualify for medical assistance, i.e., age 65 or over, blindness, perma- nent and total disability, membership in a family with dependent children under age 21, and "other." The largest group of recipients--about 43 percent--are members of families with dependent children; but most of the payments--about 47 percent--go to the aged, who comprise about 38 percent of all recipients. Bills paid for members of families with dependent children account for about 27 percent of the total. Recipients who are permanently and totally dis- abled represented a little more than a tenth of all recipients but their medical bills came to about 15 percent of all payments. The larger pro- portion of bills than of recipients found among the aged and permanently and totally disabled probably reflects the greater need for hospital and nursing home care among these groups. Average costs per person receiving these two kinds of service are known to be relatively high. Distribution of recipients and payments between money payment recipients and others.--About 65 percent of the people who had their medical bills paid in August were recipients of money payments; their bills, however, accounted for only about 45 percent of the payments to medical vendors. This disparity between the percentage of medical assistance recipients and percentage of payments accounted for by the money payment group is apparent in each "eligibility group." The reason undoubtedly is that for money payment recipients all medical bills, both large and small, must be paid from public assistance funds; the presence of some small bills will tend to bring down the average payment for the money payment group. The situation is different among those for whom money payments are not author- ized. Included in this group are first, the "medically needy only," who are likely to seek help only in paying large medical bills; and second, aged individuals in nursing homes and institutions for the mentally ill who do not need money payments so long as they remain in the home or insti- tution but for whom the monthly medical payment on the average is rela- tively high. Both these factors account for larger payments on the average for those not receiving money payments. 0 Among those aged 65 or over who got medical assistance, the 1 7 .-' Lty between recipients and payments accounted for by the money payment r.'r is the greatest. About 63 percent of the aged whose bills were na.' were money payment recipients, but their medical bills accounted for about 32 percent of payments on behalf of the aged. As indicated above, the fact that a large part of the payments for the aged is for care in nursing homes or mental institutions for people who do not get a money payment raises the share of total payments going to the non-money payment group. Noteworthy also is the fact that among the blind, the disabled, and families with dependent children getting medical assistance, the pro- portion who were money payment recipients ranLied from a high of 91 per- cent among the blind to 82 percent for the disabled and 69 percent among members of families with dependent children. As among the aged, medical bills paid for the money payment recipients in each of these groups com- prised a smaller proportion of the total than did the recipients. The higher proportion of money payment recipients among these groups than among the aged probably is accounted for by the existence of the prori-r a- of medical assistance for the aged under which the aged who did not receive money payments have been aided since 1960. No comparable program existed for the other groups of needy people until the advent of title XIX in 1966. About a fifth of the "other" recipients of medical assistance were also recipients of money payments. Their money payments would have come from general assistance or would have been maintenance payments for foster care, which are considered money payments for this report. Influence of California and New York on National Data The large, populous States will always influence national totals relating to programs that serve people. Thus, as shown in the following table, our two most populous States--California and New York--include a sizeable proportion of the people who receive money payments and of expenditures for money payments. Their even greater role in shaping the national totals for medical assistance--both recipients and payments--is reflected in the larger percentages their data comprise of medical assistance than of money payments. Similarly, the numbers of recipients represent individuals for whom one or more bills were paid during the month. For a single spell of illness, an individual may receive several medi- cal services all in the same month. If the agency received and paid all the bills during a single month, the recipient and the total amount of payments would be included in one monthly report. Usually this does not happen. Instead the receipt and payment of bills is spread over several months and the recipient and the amount paid will be reported for all the months in which one or more bills are paid. Furthermore, some people have several incidents of illness during a year and others have only one--or none. No one month--no brief period--of experience can be representative of a program's potential or of the cost of meeting such varying need. For this reason, data on consumption of medical services customarily are presented for twelve-month periods. Monthly data are collected and presented only to meet the pressing cur- rent demands for minimum information on the medical care financed from public assistance funds, primarily under title XIX. In addition, States are required also to provide mcre significant annual data on the number of people whose medical care is financed from public assistance funds and on the amounts, kinds, and costs of services provided. Description of data reported The Federal law authorizing State medical assistance programs under title XIX identifies two major groups of eligible people. First, there are those who need help to meet the cost of food, clothing, shelter, and other daily needs and who obviously also need help with their medical bills, both large and small. Medical assistance programs under title XIX, as a minimum, must cover a substantial number of those falling in this group. 2I The second group comprises those who usually are referred to as the "medically needy only," i.e., those who can manage to finance tirir daily needs on their own but who need help to pay their medical bills, especially if these are large. States establishing medical assistance programs under title XIX have the option of includingg this second *r-iup initially, but are required to extend coverage so that by 1975 a substan- tial number of such people will be eligible for comprehensive medical care when they need it. The accompanying tables classify the people who receive medical assistance (and the payments made) in two broad groups: (1) those for whom a money payment was authorized; and (2) those for whom a money payment was not authorized. The latter group includes the medicallyy needy only" (as defined above) and, in addition, those who need money payments, but can't get them because they don't meet other eligibility conditions required by the State. Some States also may count in this second group people who were authorized to receive a money payment when the medical service was provided but who were not so authorized when the vendor was paid; other States may continue to identify these people as those for whom money payments were authorized and to so report them when the medical bill is paid. The point of this description is to establish the fact that the group for whom money payments were not authorized comprises more than the "medically needy only." 3/ Furthermore, all those who are classified 2/ Coverage is mandatory for all those aged 65 or over, the blind, the permanently and totally disabled, and members of families with depenh- dent children who meet the minimum eligibility requirements speci- fied in the Federal Act under titles I, X, and XIV and/or XVI or title IV; and who are eligible for a money payment under the State's definition of a needy person. Coverage is mandatory also for chil- dren up to age 21 who except for age and/or school attendance would be eligible for money payments under title IV. 3/ The administrative and philosophical complications that would flow from attempting to identify on a monthly basis, from a group of low- income people, those who are "medically needy only" can -carcely be described. Special studies on a sample basis will be required to identify the exact number who, over a specified annual period, are "medically needy only." 6 as "money payment authorized" did in fact have this status, but the group may be more inclusive in some States than in others. Each of these two broad groups is further classified to show the basic eligibility factor that qualifies the individual to receive medical assistance. The basic eligibility factors are: (1) Age 65 or over (2) Blindness (3) Disability (4) Member of a family with dependent children under age 21 (5) Other, including children under 21 who are not properly classified under (4) and individuals 21 to 64 years of age who are not properly classifiable under (2) or (3) Groups (1) through (4) include people who receive money payments or who could qualify for a money payment under one of the federally aided money payment titles if they were "needy" within the State's definition and had to meet only such other requirements as are specified in the Federal Act. These people are "categorically related." People falling in group (5), regardless of how needy they are, would not be eligible for a money payment under a federally aided category because they fail to meet one of the basic eligibility conditions speci- fied in the Federal Act. They are, therefore, not categorically related. . Those under 21 years of age (include children in foster care and children living in families with both parents, neither of whom is disabled or unemployed, but whose incomes are low enough to permit the children to qualify for medical assistance. Federal financial participation is available in payments for these children. Individuals included in this group who are 21-64 years of age are those who do not meet the test of blindness or disability specified in the Federal Act but who are eligible for medical assistance under the State's plan. Federal financial partici- pation is not available in medical assistance furnished to this group.
