INSTRUCTIONS

This MONTHLY REPORT is mailed to Federal directors and officers in charge
of local employment offices, who are requested to verify the figures shown for
their respective States and cities and promptly report any inaccuracies.
Important.-Forms Emp. 26-A and 26-B weekly summary by local offices
should be prepared as soon as practicable after the close of each calendar week,
and forwarded without delay to the supervising office.
NOTE.-The following States report by calendar month, viz:
California. Michigan. New York.
Illinois. Missouri. Ohio.
Massachusetts.

Federal directors should submit requisitions, in duplicate, on Form
Emp. 60 for sufficient quantities of each form required during a
6-month period.
Office supplies are not furnished the cooperating services.


PENALTY ENVELOPES
Cooperating services are supplied with two sizes of envelopes-3%
by 8% inches white and 4% by 9Y inches manila. Federal directors
should submit requisitions two months in advance of date envelopes
will be needed and for quantities sufficient for a 6-month period.
Official franked envelopes can be printed only by the United States
Government.
NOTE.-Official mail to this service should be addressed "United States
Employment Service, United States Department of Labor, Washington, D. C."
Name and official title can be omitted from the address on envelopes.
