INTRODUCTION


This report has been developed for use by the University
of Florida Research community, especially those interested
in health services research. An effort has been made to re-
view the existing holdings of the University Library and to
acquire valuable additions which have been cataloged with
the Library system. The following material documents what
is available to the UF community and how to access it for
computer analysis.
This manual-includes thirteen(13) data descriptions. The
data sets described here contain information on such items as
health services utilization and expenditures vital statis-
tics, public opinion on the quality of life, housing, and
reference material on geographic bases and demography of the
population. Each data description is titled by the name of
the file; and includes a section that provides an abstract
of data contained in the. file, and a further subsection that
lists specifications for accessing the files on tape. There
is an index which should assist in guiding the inquiry to
the proper data description.
Any additions and/or corrections should be reported to
Dr. Douglas Bradham at 392-2571. This report was made avai-
lable by funding from the Division of Sponsored Research.
Most of the extant data bases residing at U of F Libraries
were acquired with library funding. Some recent additions
have been funded by the Center for Health Policy Research
under the direction of Dr. Kerry E. Kilpatrick. The gra-
cious assistance of Mr. Ray Jones at the Library's Reference
Center is acknowledged as invaluable to the preparation of
this document.
This manual is organized in notebook fashion to permit
additions to be made easily as they occur. All recipients
of the original will recieve updates periodically. Any as-

appreciated.

MAGNETIC TAPE CHECKOUT PROCEDURE AT U OF F
LIBRARY








Any requests for written or magnetic tape data files
should be placed with Ray Jones, UF Reference Librarian.
Requests should be made by calling Mr. Jones at 392-0361. A
minimum of 24 hours notice should be given with requests so
as to allow adequate time for tapes to be transferred to
NERDC and made available for retrieval.
The checkout procedure involves a physical transfer of
the tape to the NERDC tape librarian. Included in this do-
cument are the technical information items which are re-
quired to access the data tape once it is at the NERDC fa-
cilities, except for the volume or serial number. The
volume serial number must be obtained from the library dur-
ing the checkout process and is then used in the data-defi-
nition 'card' of your program. All other data-definition
parameters are included in this document.

1.1 ABSTRACT
A nationwide survey of health practices and the costs of
medical care was performed by The University of Chicago.
This is the fourth in a series of nationwide household stu-
dies concerning the utilization and cost of medical services
and insurance payments. The data were collected by the Na-
tional Opinion Research Center(NORC) for the Center for
Health Administration Studies. The survey utilizes a sample
of 72 communities plus 31 extra communities with an oversam-
pling of lower income families in large metropolitan areas.

NATIONAL AMBULATORY MEDICAL CARE SURVEYS OF
1978,1979 AND 1980


2.1 ABSTRACT: 1978 SURVEY(ICPSR 8047)
The National Ambulatory Medical Care Survey(NAMCS) pro-
vides data on ambulatory medical care rendered in physi-
cians' offices. The NAMCS is a continuous survey based on a
sample of patient visits to a national sample of office-
based physicians. The survey obtains information on the vo-
lume of office visits by age, sex and race of the patient
and by selected physician characteristics such as type of
speciality and practice and geographic location. Data de-
scribing the clinical substance of the visits include the
patient's problem or complaint, prior visit status, referral
status, time since onset of complaint, seriousness of the
problem, physician's diagnosis, diagnostic and therapeutic
services provided, disposition, and duration of visit. The
patient's problem or complaint is coded according to a re-
vised symptom classification developed specifically for the
NAMCS.
The physician's diagnosis is coded according to the
"Eighth Revision of the International Classification of Di-
seases Adapted for Use in In 1978, NAMCS sampled about
47,000 patient records from the 1,850 doctors who partici-
pated in the survey. In January of 1979, the "Ninth Revi-
sion of the International Classification of Diseases Adapted
for Use in the United States" was used to code diagnoses.

The physician's diagnosis is coded according to the "In-
ternational Classification of Diseases, Ninth Revision". In
1979, NAMCS sampled 45,351 patient records from the 1,783
doctors who participated in the survey.

2.3 ABSTRACT: 1980 SURVEY
The National Ambulatory Medical Care Survey provides conti-
nuous data from samples of patient records selected from a
national sample of office- based physicians. These national
estimates describe the utilization of ambulatory medical
care services in the coterminous United States. In 1980,
there were approximately 46,000 patient records provided by
1,870 doctors that participated in the survey. The documen-

station gives a detailed description of the survey design,
data collection procedures, and the estimation process.

1967,1975 FEDERAL AID TO FAMILIES WITH DEPENDENT
CHILDREN STUDIES


3.1 ABSTRACT: 1967 STUDY
The file was produced by the U.S. National Center for So-
cial Statistics. Demographic and financial data are pre-
sented on the members of the assistance unit in Aid to Fami-
lies with Dependent Children (AFDC). The study month was
November or December.1967. A systematic random sample was
drawn of assistance units by each state agency from a pay-
roll or similar listing for the study month. The sample
survey included all states and jurisdictions except Guam.
The minimum statewide sampling ratio required was 3 percent
or 500 cases. About 66,000 records are included. Data for
each item was provided from case records by caseworkers in
the local welfare agencies. Information is available in six
general areas:

4. AFDC- mother
5. Social Services
6. Financial Data
Restrictions: Data availability and use in no way may vi-
olate individual confidentiality. All release of the data
must be in accordance with the confidentiality requirements
as set forth in the enabling legislation.

3.2 ABSTRACT: 1975 STUDY
The data for this file were collected in May, 1975
through a systematic random sample of assistance units. The
sample was drawn by each state agency from a payroll or si-
milar listing and includes all states and jurisdictions ex-

each study item are provided from case records in local wel-

4.1 ABSTRACT
The Federal Outlays files were created for the Executive
Office of the President by the Office of Economic Opportuni-
ty. The county master file(four tapes) lists each United
States county by state and includes appropriations, pro-
grams, type of assistance, type of transactions, outlays in
dollars, benefiaries and state control of Federal outlays.

FLORIDA'S ANNUAL VITAL STATISTICS GENERAL USERS
FILE,1970-1981


5.1 ABSTRACT
These files are produced annually by the Florida Depart-
ment of Health and Rehabilitative Services Public Health
Statistics Section. They contain data on births, deaths,
and fetal deaths occurring since 1970. For each year except
1978, a consolidated birth/infant death file is available.
Each infant death record is matched with a corresponding
birth record. Statistics may be retrieved in Florida by
state aggregate, county, place, and HRS district.

1981 FLORIDA ANNUAL POLICY STUDY


6.1 ABSTRACT
The data were collected from a statewide survey of 848
Florida residents aged 18 and over. The survey was designed
to study public opinion on quality of life and issues facing
the states. Additional questions concerned the political
orientations of the state's citizens and other political
preferences. Documentation that is provided centers on the
questions pertaining to government performance and social/
health care problems.
Restrictions: Students and faculty may have complete ac-
cess. For other groups, permission is obtained through the
Policy Sciences-Program at Florida State University.

1978 U.S. ANNUAL HOUSING SURVEY


7.1 ABSTRACT
The universe of the survey, conducted by -the Bureau of
the Census for the Department of Housing and Urban Develop-
ment, is all housing units in the United States. The sample
consists of approximately 78,000 housing units selected from
the 1970 Census of Population and Housing and updated by a
sample of addresses from building permits to include new
construction and conversions.
The statistics in this file focus on characteristics of
the housing inventory. Data are provided on year the struc-i
ture was built; type of living quarters; occupancy status;
number of bathrooms and bedrooms; kitchens, plumbing and
heating facilities; and neighborhood and housing quality.
Other data reflect the frequencyand amount of mortgage,
utility, and real estate tax payments; value of property;
and characteristics of the occupants including age, race,
sex, marital status, household relationship, and income. Si-
milar data are provided for recent moves about their previ-
ous living quarters. Additional data examines energy-relat-
ed characteristics including the presence of storm windows,
storm doors, and other types of insulation; supplemental
heating equipment used; and distance, travel time, and means
of transportation from home to work for the household head.
Geographic areas included are regions, metropolitan/nonme-
tropolitan areas, inside/outside central city, selected
SMSA's, urban/rural areas, and place size outside of SMSA's.

1982 U.S. CURRENT POPULATION SURVEY ANNUAL
DEMOGRAPHIC FILE


8.1 ABSTRACT
The 1982 Current Population Survey Annual Demographic
File-is also known as the March Supplement of the Current
Population Survey(CPS). It presents data on the composition
and economic status of the population of the United States.
Comprehensive information is given on the employment status,
occupation, and industry of persons 14 years old and over.
Characteristics such as age, sex, race, marital status, fa-
mily relationship, income, educational background, and eth-
nicity are shown for each person in the household.
There are three different record types- household, fami-
ly, and person records. The household records contain the
geographic information and limited information on house-
holds. Family records contain information on family income
and other family characteristics. Person records contain
detailed income, employment, and other demographic data.
Each record contains an individual weight.
The file structure is an hierarchical file containing
three nested record types. A data base dictionary is pro-
vided. The UF Library copy of the 1982 Annual Demographic
File was provided by the Georgia State Data Center. The An-
nual Demographic Files for 1968-1982 are available through
the Florida Consortium of-Political Research from the Inter-
University Consortium of Political and Social Research. The
latter has reformated the original CPS file from the Bureau
of the Census.

1970 CENSUS CENTER OF POPULATION FILE


9.1 ABSTRACT
The tape was prepared by computing the center of popula-
tion for each county from all enumeration districts within
the county. The value computed is given in decimal degrees
of longitude and latitude to four decimal places. The file
is described as a sequential U.S. County population cen-
troid-area measurement file which also includes state sum-
mary records and a U.S. summary record.
The following data items can be retrieved: state code,
county code, county names latitude, longitudes population,
total areas total land area, water areas, and population
density.

10.1 ABSTRACT: 1978 SURVEY(ICPSR 8044)
The Health Interview Surveys(HIS), performed by the Na-
tional Center for Health Statistics, assess the amount and
distribution of illness, disability, and chronic impairment,
and the type and duration of health services received by the
non-institutionalized U.S. population. Information is col-
lected on the occurance and severity of illness and disabil-
ity and on health care use, including visits to doctors or
hospitals and operations.
Information obtained includes demographics, activities
limited by chronic or acute conditions, number of restricted
activity days, number of bed days, number of short-stay hos-
pital visits, intervals since last medical or dental visit,
current health status, and insurance coverage.
The 1978 HIS sample consists of approximately 41,000 hou-
seholds containing 110,000 persons. There are five files
which include survey results by the person, household, doc-
tor visits, hospital episodes, and health conditions.

DHPA AREA RESOURCE FILE(ARF)


11.1 ABSTRACT
The Area Resource File(ARF) is a computer-based, county-
specific health resources information system with broad ana-
lytical capabilities. It utilizes health professions and
health professions-related data that are available on a com-
patible basis for all counties in the United States. The
ARF now contains a wide range of health and socioeconomic
data( approximately 3000 data elements) of use in health
systems research. The system uses the county as the basic
geographic unit and all data contained in the system are
retrievable by individual county and by any combination of
county groupings.
The system contains data on population characteristics,
economic conditions, health professions-, health professions
training, health facilities, health care utilization, health
status, and the environment.
This file is periodically updated by the Bureau of Health
Professions. U of F has the seventh and thirteenth revi-
sions. We recommend that the researcher use the latest avai-
lable.

GEOGRAPHIC DATA FILE


12.1 ABSTRACT
The Geographic Data File(GDF) consists of three different
types of data as identified by the following record layout:
Record Type '1': Contains FIPS code, county name, and demo-
graphic data.
Record Type '2': Contains ZIPCODE, state, Post Office city
name, and FIPS county codes. All counties touched by the
ZIPCODE are included.
Record Type '3': Contains ZIPCODEstateBranch or Station
name, Post Office city name to which Branch or Station is
attached, and county codes. County codes listed are only
those touched by the Branch or Station geographic area.
Record Type '4': GSA city codes without a Post office de-
signation(available only with the GSA city code option).
Record Type '5': Unique ZIPCODES.

NATIONAL HOSPITAL DISCHARGE SURVEYS 1970-1980

The National Hospital Discharge Survey NHDS collected by
the National Center for Health Statistics provides a conti-
nuous sample of hospital discharge records. Medical and de-
mographic data are collected for calculating statistics on
hospital utilization. The survey consists of medical re-
cords data abstracted from a national sample of non-federal
short stay hospitals. The purpose of the survey is to pro-
vide more information on the utilization of the Nation's
hospitals and on the treatment and types of illness of the
hospitalized population. The National Master Facility In-
ventory of Hospitals and Institutions is the universe from
which the sample of 465-535 hospitals was taken (depending
on the year). The system used for coding medical diagnosis
is the ICDA-8. The population estimates cover the U.S. civ-
ilian, non-institutionalized population as of July 1 of the
data year. The documentation gives a detailed description
of the NHDS, the sampling design, the data collection, the
population estimates, and the measurement errors.
